NHS App and Covid Passport
The NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet or laptop. You can download the free app from the Apple or Android app stores and follow the simple on-screen instructions to set it up. You will not need to visit the practice to set up your app.
You can use the app to book and manage GP appointments, order repeat prescriptions, check your symptoms and register to be an organ donor.
From 17 May 2021 you will also be able to access your COVID-19 vaccination status through the app. It is recommended that you register with the app before booking international travel.
Do not contact your GP surgery about your COVID-19 vaccination status. We cannot provide letters showing your COVID-19 vaccination status.
If you are unable to access the NHS app but would like confirmation of your vaccination status you can call the NHS helpline on 119 and ask for a letter to be posted to you. This must be at least 5 days after you’ve completed your course of the vaccine, the letter may to take up to 5 days to reach you.

The service, run at North Manchester General Hospital, provides physiotherapy services without the need to see your GP first. You can self-refer to the service via our online form.
If you self-refer to the service and have not consulted your GP within the last 3 months regarding this problem you will be assessed by one of our First Contact Physiotherapists within 24 working hours.
If you have seen your GP, you can still self-refer via the online form and you will be referred into our physiotherapy service and placed on the waiting list until an appointment becomes available.
Within our FCP and Tier 2 service we have access to imaging and investigations, onward referrals and medication requests should this be indicated.

Due to the Global Pandemic waiting times for treatments have increased.
If you are already receiving treatment and are awaiting a follow up appointment please contact the service provider or their secretary: do not contact the practice.
If you have been referred and have not heard anything please contact the Booking and Scheduling Service, Tel: 0161 778 2233. This service is for new referrals.
Only if your symptoms are worse or your condition has deteriorated and your referral is now more urgent please complete the attached form in full stating any changes to your condition or symptoms and why the referral requires expediting.
